• The book has room for up to 15 alphabetical characters for first name (no middle name) and 15 alphabetical characters for last name, plus 8 characters for the birth date.
• There is a down-loadable form - attach this form to your e-mail or put the contents of the form in the e-mail when you order the book.
• On the first line,is where you enter the child’s first name.
• On the second line, is where you enter the child’s last name.
• On the third line, this is where you enter the child’s birth date as “mm/dd/yy.”
• Please allow approximately 3 weeks for the delivery.
